  "abstract": "Introduction and Objectives Multivariate risk prediction models are superior to category-based approaches for identifying those most likely to benefit from lung cancer screening. However, little is known about public perception of using these models to restrict screening to those at high risk of lung cancer. We aimed to understand the acceptability of specific risk-based eligibility criteria for lung screening to inform population and individual-level communication strategies.Methods As part of a larger multi-method series of studies, a qualitative study was conducted using remote semi-structured interviews with individuals (n=28) living in the UK aged 45–80 years. Participants were asked open-ended questions about their views towards targeting lung cancer screening by individual risk, the acceptability of each risk-based criterion and their anticipated reaction to disclosure of their eligibility status. Applied thematic analysis was used to interpret themes.Results Acceptability of the risk-based criteria varied according to the specific criteria. Understanding the rationale for the criteria was highlighted as important in influencing acceptability. Most participants found the criteria based on familiar and logical risk factors (age, smoking, family history) to be acceptable. Participant views around ethnicity and educational level as eligibility criteria were more complex due to lack of understanding about the relevance of these factors in influencing lung cancer risk with a minority of participants expressing concern that these criteria may be perceived as unfair and discriminatory. The exclusion of people who have never smoked but felt at risk due to passive smoking was a concern for some participants. Ineligibility evoked feelings of disappointment and worry, and there was misperception that ineligibility may signify good lung health.Conclusions This is the first study to seek the UK public’s perspectives on the acceptability of targeting lung cancer screening by individual risk. Individuals were broadly accepting of risk-based eligibility criteria for lung screening where there was evidence linking the criteria to increased risk of lung cancer. Lack of understanding about the risk factors for lung cancer development as well as misperceptions around the diagnostic capability of the risk eligibility criteria highlight the need for widespread public communication before direct invitation to lung screening.",
