  "abstract": "Background SGLT-2 inhibitors are used extensively in the management of diabetes and heart failure given their powerful anti-hyperglycaemic and diuretic effects. Recent research has shown the multifaceted immunomodulatory and anti-inflammatory effects of SGLT2-inhibitors with recent research suggesting favourable outcome in patients with COPD.Objective Our aim was to conduct a retrospective observational study into the relationship between SGLT-2 inhibitors and their effects on length of stay, frequency of admissions and use of acute NIV in patients admitted with an acute exacerbation of COPD between April 2022 and April 2024.Methods Patients with COPD confirmed by spirometry and discharged with a primary diagnosis of ‘exacerbation of COPD, infective or non infective’ were identified from electronic records.Patients with concurrent diagnoses during the admissions such as primary lung cancer and its complications, acute ischaemic heart disease or acute heart failure, complications from acute renal failure were excluded.Results 627 patients, representing 1197 admissions, were identified. 32 patients received SGLT-2 inhibitors. Those receiving SGLT-2 inhibitors were associated with a 26% reduction in admission duration (CI 0.62–0.87; p≤0.001), accounting for age, sex, FEV1 and concurrent diagnosis of congestive heart failure and Type 2 Diabetes.There was no significant reduction in the frequency of admissions or in the use of acute non-invasive ventilation in the SGLT-2 inhibitor cohort.Conclusion Our study has a revealed that SGLT-2 inhibitors significantly reduce the duration of in-hospital admission for those with COPD. However, that there was no effect on the frequency of admissions or reduction acute NIV in this group. Nonetheless, this offers a tantalisingly new direction in the management of COPD patients.",
