  "abstract": "Background Hospitalisations related to interstitial lung disease (ILD) are associated with significant morbidity and healthcare burden. While seasonal variation has been well documented in other respiratory conditions, its influence on ILD-related hospital admissions remains unclear. 1 This study aimed to assess seasonal trends in admission frequency, length of stay (LOS), and in-hospital mortality among patients admitted with ILD.Methodology This retrospective study was conducted at a single UK tertiary ILD centre, analysing all primary-coded ILD related hospitalisations between December 2017 and December 2024. Seasons were defined as winter (Dec-Feb), spring (Mar-May), summer (Jun-Aug), and autumn (Sep-Nov). Outcomes included the number of admissions per season, median LOS, and in-hospital mortality. Statistical comparisons were performed using the Kruskal-Wallis and chi-squared tests.Results A total of 1701 admissions were included (mean age 71.7, SD14.0 years; 50.5% male). The seasonal distribution of admissions was: 432 (25.4%) in winter, 446 (26.2%) in spring, 379 (22.3%) in summer, and 444 (26.1%) in autumn (p=0.213). Median LOS was 6 days (IQR 2–11) in winter, 5 days (IQR 2–11) in spring, 6 days (IQR 2–11) in summer, and 6 days (IQR 3–12) in autumn (p=0.232). In-hospital mortality was11.1% in winter, 9.4% in spring, 7.92% in summer, and 10.1% in autumn (p=0.392). No statistical significant seasonal differences were observed in admission frequency, LOS, or mortality.Abstract P19 Figure 1Conclusion This large single-centre study found no evidence of seasonal variation in ILD-related hospital admissions, length of stay, or in-hospital mortality. These findings suggest that seasonal factors may play a limited role in hospitalisation risk, supporting consistent year-round planning of ILD services.
