  "abstract": "Introduction Robotic-assisted bronchoscopy (RAB) enhances navigation, reach, and diagnostic accuracy for peripheral lung nodule biopsies. Prospective studies have demonstrated a diagnostic yield of 86–93% with low complication rates. Whilst available in >500 USA centres, it’s adoption across Europe is at an early stage. In this abstract we report outcomes from the first 100 RAB cases performed at a UK trust.Methods Our RAB procedures are performed between two interventional bronchoscopists, under general anaesthetic, in a hybrid theatre space. The team uses the Ion® Endoluminal Robotic Bronchoscopy System ( Intuitive Surgical), integrated with the Cios-Spin mobile imaging system utilising cone-beam CT (Siemens). A prospective database is used to collect patient, nodule, procedural details and clinical outcomes. Here, we report outcomes from the first 100 cases performed between September 2024 and April 2025.Results Mean age was 68 (range 47–88; 50% female). Median lesion size was 22 mm, reducing from 24.5 mm in the first 50 cases to 17.5 mm in the second 50 cases. 87% of lesions were in the middle-to-outer thirds of the lung.Diagnostic accuracy was 94%, with a diagnostic yield of 86% (strict criteria). Tool-in-lesion was achieved in 97% of cases. There have been no major complications and no pneumothoraces. 9 patients had minor bleeding, managed endobronchially, and no acute clinical admissions.Two thirds (n=65/100) of cases did not have a safe alternative biopsy option, defined as either not amenable, or extremely challenging and high risk for a CT-guided biopsy. Of these, 49/65 had a lung cancer diagnosed, from which 32/49 had surgical resections. 14/65 patients had a true negative diagnosis, all of whom had suspected lung cancer radiologically. 96% of samples requiring further molecular analysis had adequate material.25% of cases also had concurrent staging EBUS-TBNA, of which 96% (n=24/25) confirmed adequate N0 disease.Conclusion RAB can be implemented within NHS services in a safe and effective manner. It can be focussed to target lesions not amenable to other forms of biopsy and be combined with staging EBUS-TBNA when indicated. Further prospective work is needed to define its exact role in optimal lung cancer pathways and determine cost-effectiveness within the NHS.",
