  "abstract": "Introduction Glucagon-like peptide-1 (GLP-1) receptor agonists are novel antihyperglycemic agents that have demonstrated benefits in cardiovascular and renal disease. In murine models of obstructive lung disease, GLP-1 agonists have been shown to reduce airway inflammation and hyperresponsiveness. In humans, some observational studies have shown an association between GLP-1 agonist use and reduced exacerbations of chronic obstructive pulmonary disease (COPD), as well as a lower risk of non-invasive ventilation, invasive mechanical ventilation, and bacterial pneumonia. We examined the effects of GLP-1 agonists on mortality in a large sample of patients with COPD.Methods In this retrospective case-control study, we identified patients with COPD from a database at our institution. We then identified those who had received a GLP-1 agonist after their COPD diagnosis. Data are presented as median with interquartile range, and a Kaplan-Meier curve was used for mortality analysis.Results We identified 186,985 patients with COPD who had not received a GLP-1 agonist and 1,877 patients who had. The treatment group was followed for a median of 1,247 days (interquartile range: 667 to 2,015), compared to 349 days (174 to 606) in the control group.The median age in the GLP-1 group was 68.8 years versus 60.3 years in the control group. Females comprised 49% of the GLP-1 group and 48% of the control group. In the GLP-1 group, 80% of patients were obese, with a mean BMI of 32.5, compared to 26.5% in the non-GLP-1 group. Additionally, 89% of patients in the GLP-1 group had type 2 diabetes mellitus, compared to 28% in the control group.Patients who received a GLP-1 agonist had improved survival at 1, 5, 10, 30, and 60 months compared to those who never received a GLP-1 agonist (p < 0.001; hazard ratio: 7.13, 95% CI: 5.66–8.99).Abstract P158 Figure 1Discussion Our study showed that GLP-1 agonists were associated with improved survival in patients with COPD. This may be related to improved cardiovascular mortality in these patients. Further studies are needed to evaluate GLP-1 agonists as potential therapeutic options for patients with COPD and to elucidate the mechanisms underlying this association.",
