  "abstract": "Background Aerobic and resistance training are core components of a pulmonary rehabilitation (PR) programme (Man et al, 2024). A knee extension one repetition maximum (1RM) test measures the maximal strength of the quadriceps, however due to equipment available it is common in PR to measure a 1RM indirectly via an estimated 1RM (E1RM), using repetition testing at submaximal loads. Loading the lower limb muscles effectively even sub maximal levels can be a challenge without specialist gym equipment. As many PR services run in community venues, this equipment is often not available. The aim of this project was to see an E1RM squat exercise would it lead to similar improvements lower limb strength following a PR programme the commonly used knee extension E1RM does.Method Patients who attended and completed a twice weekly PR programme for 8 weeks between January and May 2025 were included. Assessment as per PR guidelines were performed pre and post, including incremental shuttle walk test (ISWT) or six-minute walk test (6MET) and estimated 1RM leg extension. The Brzycki equation was then used to give an estimate 1RM (E1RM). All data was analysed to see if there were statistically significant changes and correlation between outcome measure results.Results During the period 11 patients completed the PR programme table 1 outlines their demographics. There was a statistically significant change in average pre to post muscle strength score measured via the squat E1RM (p=0.003), with all 11 patients improving.While was a statistically significant and strong positive correlation between pre and post E1RM squat and leg extension scores (table 1), the correlation between change in scores was weakly positive (r=0.283), only suggesting a possible relationship that if one improves, so does the other.Pre to post distance in the ISWT was statistically significant (p=0.28), however this did not correlate with E1RM squat scores, with only a moderate correlation between the results. There was not enough data for the 6MWT.Abstract P45 Table 1Patient Demographics and outcomesConclusion This was a small study, however the results indicate that E1RM squat test, could be a useful alternative to an E1RM leg extension in a community PR programme.",
