  "abstract": "Background Biologic therapies have shown great promise in asthma clinical trials over the past decade. It is important to understand their impact on patients’ quality of life, lung function, steroid usage and healthcare utilisation when deployed at scale in a real-world tertiary service.Methods A retrospective anonymised observational cohort study was undertaken using registry data from the East of England Severe Asthma Network from the past decade comparing patients before and after starting an asthma biologic drug. All consecutive adult patients commenced on a biologic (2013-present) were included if both a baseline (pre-treatment) and at least one follow-up (on treatment) clinic visit had occurred. Where more than one follow-up dataset was available, the most recent was used. Clinical data included steroid usage, hospital attendances, pulmonary function tests and asthma-specific patient-reported outcome data - Asthma Control Questionnaire (ACQ6).Results Data was included from 806 adults: 59.8% female, 81.4% White British ethnicity, mean baseline BMI 31.3 kg/m 2 (SD7.5). There was a mean of 3.3 years between baseline and follow-up data. Patients were prescribed: benralizumab n=442, dupilumab n=69, mepolizumab n=146, omalizumab n=51, tezepelumab n=69, not recorded n=29. 49.3% of patients were taking maintenance oral corticosteroids at baseline compared to 25.5% at follow-up (Fisher’s test; p<0.0001). There was a median of five exacerbations requiring a rescue course of oral corticosteroids in the 12-months preceding baseline compared to a median of zero annual exacerbations at follow-up (Mann-Whitney test; p<0.0001). 29.3% of patients had at least one acute hospital attendance for asthma in the 12-months prior to baseline compared to 4.4% at follow-up (Fisher’s test; p<0.0001). The median averaged ACQ6 score was 2.8 (IQR 1.8–3.8) at baseline compared to 1.0 (IQR 0.5–2.0) at follow-up (Mann-Whitney test; p<0.0001) (figure 1). The median FEV1 did not differ (2.24L vs 2.27L, p=0.14).Abstract P196 Figure 1Conclusions Following the initiation of asthma biologic therapies, acute hospital attendances, health-related quality of life and both acute and chronic corticosteroid usage improved. Whilst acknowledging both the limitations of this unblinded, non-controlled observational study but also the strengths of real-world clinical service data, the positive impact of asthma biologics on multiple patient-centred clinical endpoints is profound.",
