  "abstract": "Background Bronchoscopic lung volume reduction (BLVR) using endobronchial valves (EBVs) is associated with post-procedure pneumothorax in up to 30% of cases. Immediate lobar collapse is desirable but not always radiologically evident intraoperatively. This pilot case series evaluates the utility of intraoperative bedside thoracic ultrasound in identifying lobar collapse and ruling out pneumothorax in real time during EBV procedures.Methods Eight patients underwent BLVR with Zephyr valves. Intraoperative bedside thoracic ultrasound was performed to assess diaphragm elevation and pleural integrity. All ultrasound findings were confirmed with post-procedure chest radiographs. Chartis assessments were negative in all cases.Results The mean age was 67.1 ± 10.1 years; 4 patients (50%) were female. A median of 4 valves (range 3–5) were implanted per patient. Lobar collapse, indicated by diaphragmatic elevation or visible lung sliding loss, was observed in 6 of 8 patients (75%) via ultrasound. The remaining 2 showed no significant sonographic changes, which was confirmed by CXR. No pneumothorax was detected intraoperatively or post-procedure. One case showed blunting of the costophrenic angle.Conclusion Intraoperative bedside thoracic ultrasound is a feasible, non-invasive adjunct to EBV procedures, enabling immediate evaluation of lobar collapse and exclusion of pneumothorax. Its integration into BLVR workflow may improve procedural safety and clinical decision-making.",
