  "abstract": "Background Characteristics of lung cancer in never-smokers in the UK are poorly understood. We examine outcomes of never-smokers referred to lung cancer MDT and report on individuals subsequently diagnosed with lung cancer.Methods We reviewed electronic health records of adults referred to lung cancer MDT at University College London Hospital between 2019 and 2025. Individuals diagnosed with lung cancer were recruited to the OLIVE study funded by NIHR (NIHR303686) and Ruth Strauss Foundation. Demographic and clinical data were compared to National Lung Cancer Audit data.Results Of 4,765 individuals discussed at MDT ( figure 1), 75% (n=3,574/4,765) were excluded due to ever- (n=3,569) and unknown (n= 5) smoking status.35% (n=417/1,191) of never-smokers discussed at MDT were diagnosed with lung cancer. 51% (n=213/417) were recruited to OLIVE. 71% (n=151/213) were coded as never-smokers in electronic health records while 29% (n=62/213) had documentation in clinical notes only. Median age at diagnosis was 67 years (IQR 57–79). 71% (n=152/213) were female.37% (n=80/213) were diagnosed with early-stage (30%,n=66/213 Stage I; 7%,n=14/213 Stage II), and 62% with late-stage disease (16%,n=34/213 Stage III; 46,n=99/213 Stage IV). One-year survival was 74% (n=158/213).Comparatively, national data1 on all adults with lung cancer report median age at diagnosis as 74.2 years (IQR 67–80, p<0.001) and 50% as female (p=0.003). 28% are diagnosed at Stage I, 8% Stage II, 12% Stage III and 43% Stage IV (p=0.925). One year-survival is 46% (p<0.001).44% (n=520/1,191) of never-smokers had clinically significant non-malignant intrathoracic findings; this included indeterminate pulmonary nodules (n=153), infection (n=79), thymic lesions (n=66) and interstitial lung disease (n=44). 21% (n=254/1,191) were diagnosed with other malignancies including haematological (n=41), mesothelioma (n=33), gastrointestinal (n=31), gynaecological (n=31), urological (n=30) and breast (n=28).Abstract P170 Figure 1Flow diagram of individuals referred to lung cancer multi-disciplinary team (MDT) and reasons for exclusion from OLIVE studyDiscussion In our cohort, 25% of individuals referred to lung cancer MDT were never-smokers. 35% of these were diagnosed with lung cancer. Never-smokers with lung cancer are likely to be female and younger. In our small cohort, despite having similar stage, survival is significantly better.
