  "abstract": "Background The UK’s Lung Cancer Screening Programme aims to facilitate earlier lung cancer detection in adults aged 55–74 with a smoking history. Social deprivation is strongly correlated with increased lung cancer incidence. The East London boroughs of Tower Hamlets, Barking and Dagenham, and Newham are among the UK’s most deprived areas, whilst Havering is near the national average for deprivation. This study evaluates outcomes of Lung Cancer Screening in these boroughs from 2023 to 2025.Methods We conducted a retrospective analysis of the programme’s data from 18th April 2023 to 18th March 2025 across the specified East London boroughs. Scan outcomes were triaged into different pathways based on urgency for clinical action.Results 45,123 patients were invited during the designated time period with an uptake of 58.6% (26,433 patients) completing the health checks. A total of 12,415 low dose CT scans were performed involving 10,251 patients. 2753(22%) required follow-up for lung nodules closer than the routine 24-month interval. Non-urgent referrals to secondary services were made for 716 (5.8%) patients due to other pulmonary or extra-pulmonary findings. Urgent clinical action was required for two patients (large pneumothorax, severely dilated ascending aortic aneurysm). A total of 322 (2.6%) scans had concerning findings necessitating urgent review, including potential lung cancer, severe active Tuberculosis, dilated aorta, unilateral pleural effusion, and other extrapulmonary lesions suspicious for malignancy. 117 patients had histologically confirmed lung malignancy, with 86 (74%) diagnosed at an early stage (Stage I or Stage II). 34% of the patients with confirmed lung cancer were under the age of 65, 59% were male, and only 24% were from an ethnic minority background. The distribution of confirmed lung cancer cases by borough was: Havering 7 (0.7% of patients from borough scanned), Newham 24 (1.0%), Barking & Dagenham 29 (1.1%), and Tower Hamlets 55 (1.4%).Conclusion Lung cancer screening in East London effectively identified a significant number of lung cancer cases, predominantly at early stages, within a socially diverse and often deprived population. Further analysis is warranted to explore how specific social deprivation indices correlate with participation rates, referral pathways, and stage at diagnosis.",
