  "abstract": "Introduction Pulmonary hypertension (PH) in patients with interstitial lung disease (ILD) is associated with increased morbidity and mortality. Current guidelines lack an evidence-based approach for detection of PH in the setting of ILD. The PHINDER study ( NCT05776225) aims to identify clinical features associated with PH in patients with ILD to develop a diagnostic risk tool.Methods PHINDER is a multicenter, prospective study enrolling 300 ILD patients with features suggestive of PH. Participants underwent routine assessments (physical exam, pulmonary function tests, high-resolution computed tomography, echocardiogram) followed by right heart catheterization (RHC). An interim analysis conducted in January 2025 used descriptive statistics to summarize baseline features. Univariate logistics regression analysis was performed to assess predictors for PH, defined as mPAP >20 mmHg, PVR >2 Wood Units (WU), and PAWP ≤ 15 mmHg.Results Of the 143 patients enrolled, 128 underwent RHC. Participants were primarily white (82%), male (56%) with mean age 70±13 years and mean time from ILD diagnosis 4.1±4.2 years. Of these, 66 (52%) had PH-ILD with mPAP 28.3±5.3 mmHg and PVR 4.3±1.9 WU. Findings on lung function (DLCO, DLCO%,% predicted FVC/% predicted DLCO), serum creatinine, echocardiography (TAPSE/PASP), and CT (pulmonary artery enlargement, pulmonary artery/aorta diameter, and RV/LV diameter >1) were associated with a diagnosis of PH.Discussion/Conclusion We identified several features from commonly used clinical assessments that were predictive of PH in ILD. Results from the fully enrolled study will be used to develop a multicomponent screening tool to aid in detection of PH in ILD.",
