"abstract": "Rationale Remission in asthma is generally agreed to be a conjugate of sustained symptom control, absent exacerbations, no systemic corticosteroid use and stable lung function. The impact of triple inhaled therapy versus dual therapy for achieving asthma remission is unknown. Methods We conducted a post-hoc analysis of the TRIMARAN (Triple versus medium strength of ICS/LABA) and TRIGGER (Triple versus high strength ICS/LABA) studies [Virchow JC et al, Lancet 2019] to assess the proportion of study participants who met the following criteria of remission on treatment: no severe asthma exacerbations and systemic corticosteroids over 52 weeks, ACQ-5 score < 1.5 at Week 26, 40 and 52, and change from baseline in trough FEV1 ≥ 100 mL at Week 52 OR a 2hr post-dose FEV1/FVC ratio > 0.7 at Week 52. Odds ratios (OR; 95% CIs) were calculated from a logistic regression model. Results In TRIMARAN, 174 (30.2%) and 147 (25.6%) patients met all remission criteria while on treatment with medium dose BDP/FF/GB (400/24/50 mcg/day) and BDP/FF (400/24 mcg/day), respectively (OR=1.268; 95% CI (0.976, 1.647), p=0.075). In TRIGGER, 165 (28.9%) and 107 (18.7%) patients met the criteria while on high-dose BDP/FF/GB (800/24/50 mcg/day) and BDP/FF (800/24 mcg/day), respectively (OR=1.776; 95% CI (1.344, 2.346), p<0.001).Conclusions A greater proportion of patients achieved clinical remission while on treatment with BDP/FF/GB triple therapy than with BDP/FF. The difference was particularly noted in patients whose asthma was previously uncontrolled on high-dose ICS/LABA treatment who were stepped-up to high-dose BDP/FF/GB.",
