  "abstract": "Introduction and Background Accurate diagnosis of peripheral lung lesions can be challenging and requires multiple bronchoscopy techniques. The Lung Vison Navigation system (LV) utilises augmented fluoroscopy to facilitate localization of lesions. This study compares diagnostic outcome of mini probe Radial EBUS (MP) alone versus with combination of MP with LV.Methodology This retrospective study was performed on 71 patients who underwent sampling of peripheral lung lesions before and after introduction of LV in June 2023. MP group included 39 patients who underwent sampling before introduction of LV and LV group included 32 patients who underwent sampling during the first few months after introduction of LV.Results All the patients underwent procedures under general anaesthesia. Diagnostic yield was higher in LV group compared to MP group (81% vs. 64%, OR 1.23, 95% CI 0.45–3.37, p = 0.682). Significantly higher numbers of samples were adequate for molecular markers in LV group ((78% vs. 54%, χ2 = 4.54, p = 0.033). Lesser number of patients from LV group required further diagnostic procedures compared to MP group, although there was no statistical significance for this observation. There was one pneumothorax, and a minor bleeding reported from MP group, both of which were conservatively managed. There was one case of atrial fibrillation reported from LV group which was manged medically.Abstract P217 Table 1Comparison of mini probe R-EBUS group and lung vision navigation groupConclusion This study was conducted during the period immediately after introduction of Lung Vision Navigation. Combining LV with REBUS mini probe guided biopsy led to improved diagnostic yield, reduction of need for repeat procedures as well as significant increase in sample adequacy for molecular testing, with minimal complications.
