  "abstract": "Background Human papillomavirus (HPV) vaccination is strongly recommended for Men who have Sex with Men (MSM), including those living with HIV and those using HIV pre-exposure prophylaxis (PrEP). However, coverage remains suboptimal. Understanding differences in vaccination uptake and factors associated with hesitancy is crucial to inform targeted prevention strategies.Methods HPV vaccination status of all MSM attending our PrEP Clinic was assessed using the regional electronic health records system. Data on other recommended vaccinations (hepatitis B [HepB], recombinant zoster [RZV], meningococcal ACWY [MenACWY], pneumococcal conjugate [PCV], mpox modified vaccinia Ankara–Bavarian Nordic [MVA-BN], quadrivalent influenza [IIV4], mRNA COVID-19) were also collected. The same data were retrieved in an age- and sexual orientation-matched group of people living with HIV (PWH) (1:2 ratio). Vaccination prevalences were compared by HIV status and HPV vaccination status using the Chi-Square test. A Cox regression model was applied to identify sociodemographic factors (HIV status, older age, lower education, unemployment, distance from the Infectious Diseases [ID] Clinic, no vaccination in the last three years) associated with higher Odds Ratio (O.R.) of not being vaccinated for HPV with 95% Confidence Interval (C.I.).Results One-hundred and eighty-two MSM were enrolled. Their general features are reported in table 1. The overall prevalence of HPV vaccination was 44% and was remarkably higher among PrEP users than in PWH (76% vs 30% p<0.001). Vaccination, in all 81 cases performed with 9-valent recombinant HPV vaccine, had been received free of charge and in the same HIV/PrEP Clinic by the 63% of participants and had been suggested in almost all cases by the ID Specialist (74/81 subjects, 97%). While high prevalence of HepB was observed (73%), the overall rates of adherence to other non mandatory vaccinations were overall low, with lower rates reported for RZV (12%), MenACWY (10%) and COVID-19 (5%). Significantly higher prevalence of adherence to these vaccinations was reported among PrEP users compared to PWH and in subjects vaccinated vs non vaccinated against HPV, as reported respectively in tables 2 and 3. Seventy-three subjects (43%) affirmed to have not received any vaccination in the last three years. At multivariate analysis, having received other vaccinations in the last three years resulted strongly protective against HPV vaccination hesitancy (O.R. 0.01, 95% C.I. 0.00-0.05, p<0.001).Conclusions HPV vaccination coverage remains suboptimal among MSM, especially PWH, while PrEP users show higher uptake. Engagement with other vaccination programs strongly predicts HPV vaccination acceptance. Implementing dedicated vaccination pathways—preferably within the same clinic used for PrEP or HIV care—and optimizing counseling among PWH seem vital to improve vaccines uptake.Abstract P223 Table 1–3",
