  "abstract": "Background This study aims to assess changes in vaccination coverage (VC) among people living with HIV (PLWH) in the Lombardy Region in 2025, following the implementation of a vaccination coverage monitoring system in 2024.Materials and Methods Administrative data were used to identify PLWH as individuals with ≥1 antiretroviral (ARV) prescription (excluding TDF/FTC) or ≥1 HIV viral load record. Each person was assigned to an infectious diseases clinic (IDC) based on their latest prescription in the calendar year. Two cohorts (2024, 2025) were defined: PLWH identified between 1 January and 31 December of each year, with VC assessed on 31 December; thus, VC variation reflects changes occurred due to vaccination performed in 2025. VC was stratified by IDC, sex and age group (0–49, 50–64, ≥65). The analysis included vaccines recommended for PLWH by regional guidelines. Vaccination data were obtained from the regional registry and included all doses recorded in the Lombardy system, irrespective of the place of administration and setting.Results Table 1 summarizes age groups and sex distribution for the 2024 (n = 28,364) and 2025 (n = 29,322) cohorts. Figure 1 presents vaccination coverage (VC) for 2024 and 2025, while figure 2 shows the changes in VC observed in 2025.The changes in 2025 for selected vaccines are reported below, together with the minimum and maximum variations in first-dose coverage across IDCs.Herpes zoster (HZ): +6.8% (3.3% - 15.7%)Pneumococcus (PC): +3.8% (1.3% - 10.0%)HPV: +4.5% (2.1% - 11.5%)Haemophilus influenzae b: +2.7% (0.2% - 13.3%)Hepatitis A (HAV): +1.4% (0% - 4.6%)VC varied across IDCs in the overall population.For PC, coverage in 2025 ranged from 24.7% to 71.5% overall across IDCs. In the ≥65 age group, variability remained wide, ranging from 33.3% to 83.2% in men and from 37.5% to 80.6% in women.For HZ, overall coverage ranged from 11.5% to 60.6% across IDCs. Among individuals aged ≥65 years, coverage ranged from 15.9% to 71.4% in men and from 6.9% to 46.2% in women.For HAV, overall coverage ranged from 8.3% to 28.9% across IDCs. In the 0–49 age group, coverage ranged from 13.7% to 42.7% in men and from 7.2% to 23.9% in women.Conclusions Overall VC improved in 2025 but substantial heterogeneity was observed across IDCs likely due to implementation of targeted strategies to enhance vaccination uptake, such as on-site vaccination during visits, altogether showing that IDCs play a pivotal role in promoting vaccination. VC varied by sex and age group. Higher HAV coverage among younger men likely reflects targeted recommendations (e.g., men who have sex with man), whereas other differences - such as pneumococcal VC in ≥65 - showed heterogeneity across IDCs despite age-based vaccination opportunities independent of HIV status. Establishing a monitoring system for VC among PLWH was important to assess immunization levels, increase awareness among IDCs and evaluate the impact of interventions.Abstract OC75 Figure 1–2 and Table 1",
