  "abstract": "Background Implementation of long-acting injectable antiretroviral therapy (LAI-ART) may significantly improve the quality of life of people living with HIV (PLWH). Patient-reported outcomes (PROs) are key tools to assess this impact. This study evaluated satisfaction with LAI-ART in a cohort of PLWH.Methods This prospective single-center study enrolled PLWH who switched to bimonthly LAI-ART with cabotegravir (CAB) and rilpivirine (RPV) between January 2023 and October 2024, with follow-up through November 2025. PROs were assessed using the EQ-5D (Quality of Life and Health Status Scale), the HIV-TSQs (HIV Treatment Satisfaction Questionnaire, status version), and the Resilience Scale (RS) before LAI-ART initiation (T0) and after 6±1 months (T1) and 12±1 months (T2).Results Overall, 60 individuals were included; 49 (81.6%) were male, with a mean age of 47.1 years (SD 11.0). All participants completed baseline PROs assessments; 56 completed T1 and 52 completed T2 (8 discontinued LAI-ART). At T0, mean EQ-5D scores were very high, ranging from 1 to 1.3 across all domains, with a mean self-rated health status of 84.8/100. The HIV-TSQs showed the lowest baseline scores in items related to compatibility of ART with lifestyle (mean 5.2/6) and willingness to continue current ART (5.05/6). Baseline RS scores were also high, exceeding 5.5 in all measured items ( figure 1).Regarding HIV-TSQs, scores increased across all domains at both follow-up visits. The side effects item showed a mean score of 5.67/6 at T1 and 5.8/6 at T2. The most pronounced improvements over time were observed in domains related to treatment burden, convenience, flexibility, and fit with lifestyle. Most participants reported high satisfaction with continuing LAI-ART (5.94/6 at both T1 and T2) and would recommend it (6/6 at T1, 5.94/6 at T2).EQ-5D quality-of-life scores remained high throughout follow-up. Self-perceived health status showed a modest improvement at T1 (86.49/100) and a slight decline at T2 (84.64/100), remaining comparable to baseline. RS scores remained substantially stable over time, with minimal variability. However, interest in the surrounding environment progressively increased, while energy for daily activities, after an initial improvement at T1, declined at T2.Conclusions Quality of life, resilience, and treatment satisfaction scores increased and remained high in the first year following LAI-ART initiation.
