  "abstract": "Background People with HIV (PWH) are at increased risk of anal cancer compared with the general population. The International Anal Neoplasia Society (IANS) recently published guidelines to standardize anal cancer screening and identify populations at risk. Aim of the study was to assess the impact of the IANS consensus guidelines release on anal cancer screening uptake in a real-life cohort of people with and without HIV and to evaluate concordance between clinical practice and recommendations.Methods Cohort study with retrospective analysis of prospectively collected routine clinical data from people with and without HIV, followed at the Infectious Diseases Unit of San Raffaele Hospital, Milan, Italy.Eligible participants were those who had received anal cancer screening during one of two study periods: before the release of the IANS Guidelines (1st January 2023–31st June 2024) and after their release (1st July 2024–31st December 2025). A six-month lag period was included to account for waiting-list delays.Demographic, clinical, and laboratory data were collected from the CSL-HIV Cohort database. Baseline characteristics between the two periods were compared using the Wilcoxon rank-sum test for continuous variables and the chi-square test or Fisher’s exact test for categorical variables, as appropriate.Results A total of 446 and 624 individuals were screened in the first and second periods, respectively, corresponding to 455 and 647 screening examinations, indicating increased uptake. Participants screened in the first period were significantly younger. Among PWH, immunovirological characteristics were comparable between groups in terms of CD4 nadir, viral suppression, and prior AIDS diagnosis; however, immunological parameters were modestly improved in the second period.After guideline release, a higher proportion of women (4.8% of those screened) and heterosexual individuals underwent screening. In contrast, pre-exposure prophylaxis (PrEP) users and individuals with a history of sexually transmitted infections were more frequently screened in the first period. Individuals with prior anal condylomata and those previously tested for HPV were more commonly screened in the second period (table 1).Overall, concordance with screening recommendations significantly improved following guideline release. In the first period, 367/446 individuals (82.3%) met the recommended indications, compared with 586/624 (93.9%) in the second period (p< 0.0001). MSM with HIV represented the most frequently screened population in both periods (figure 1).Conclusions Our findings provide real-world evidence of the impact of the IANS recommendation publication, demonstrating rapid implementation in clinical practice alongside a parallel increase in screening uptake, which plausibly reflect increased clinician awareness and sensitization to the indication, particularly given that no additional appointment capacity was made available within the service.Abstract OC62 Figure 1Abstract OC62 Table 1",
