Strano| Unit of Infectious Diseases, San Raffaele Scientific Institute, Milano, ItalyBackground Despite impressive improvements in the management of HIV infection, people with HIV (PWH) still present a high burden of physical and psychological comorbidities. Identifying health needs in persons under antiretroviral treatment (ART) is critical for improving healthy life expectancy and robust ageing. The Mental Health Group of Coalition HIV developed a multidimensional screening questionnaire, and a prospective study (MUSA) was designed for its validation.Material and Methods The screening tool comprises 23 questions covering 12 domains (cognition, functional capacity, depressive and anxiety symptoms, sleep quality, quality of life, biological/nutritional status, socioeconomic factors, stigma, comorbidities, polypharmacy, and alcohol and substance use). It can be completed in about 5 minutes, and participants rated each item on a 1–4 scale (best to worst outcome). The self- or operator-administered confirmatory tools (with the used thresholds) are shown in table 1. Data are shown as numbers (percentage) or median (interquartile range) and assessed through non-parametric tests.Results We enrolled 202 participants. They were mostly male (128, 63.4%) with a median age of 58 years (49-65). They presented a CD4 cell count of 703 cells/mm3 (510-967) and HIV RNA <50 copies/m in the majority (190, 95%), despite many years of infection (19, 9-30). The domains most commonly identified as altered at the screening test were stigma (138, 70.4%), socioeconomic status (125, 63.8%), and sleep (53, 26.9%). Confirmatory tests identified altered sleep (101, 51.5%), socioeconomic (85, 43.1%), and quality of life (75, 38.1%) domains; no participant had an altered functional domain at the confirmatory test. We observed a significant direct correlation (p<0.001, R2=0.364) between the total abnormal domains at screening (2, 2-4) and at confirmatory tests (2, 1-3): the screening test had a sensitivity and specificity of 96.3% and 11.8% in identifying at least one altered domain at confirmatory tests. The performance of the screening tool varied largely according to the domains: it was highly predictive for alterations in depression, anxiety, multimorbidity/polypharmacy, socioeconomic, addictions and stigma, but with varying sensitivity and specificity within each domain ( table 1); cognition and functional domains were not correctly identified.Conclusions We developed a new brief screening test that can help identifying patients with higher health needs. A good correlation with the burden of altered domains was found as well as a fare prediction of alterations in depression, anxiety, multimorbidity/polypharmacy, socioeconomic, addictions and stigma.
