  "abstract": "Transcranial Doppler ultrasound has long been established as a valuable, non-invasive tool (in both research and clinical practice) for detecting microembolic signals in patients with acute ischaemic stroke. 1 2 Microembolic signals represent circulating embolic material within the intracranial arteries and act as a surrogate marker of ongoing embolic activity, often arising from unstable atherosclerotic plaque or a cardiac thrombus. Beyond their diagnostic role, microembolic signals carry prognostic value, being associated with an increased risk of early stroke recurrence1 and, more recently, with poorer functional outcomes, particularly in stroke caused by atrial fibrillation.3 We present a patient in whom the detection of microembolic signals in the acute phase of stroke directly influenced clinical management.",
