  "abstract": "A 48-year-old woman was referred to our women-dedicated stroke clinic with recurrent embolic strokes. She had no traditional vascular risk factors. However, 20 years previously, she had developed Raynaud’s phenomenon and thrombotic thrombocytopenic purpura during pregnancy. In 2008, when investigated for migraines, she was found to have chronic right frontal and cerebellar infarcts and started aspirin 80 mg/day. In March 2023, she developed an acute embolic stroke with left M3 branch occlusion, treated with intravenous thrombolysis (tenecteplase). A comprehensive stroke workup gave normal or negative results, including Holter monitoring, transthoracic and trans-oesophageal echocardiograms, CT angiogram of the head and neck, MR scan of brain, lumbar puncture and prothrombotic serum markers (anticardiolipin, anti-β2 glycoprotein I antibodies, lupus anticoagulant, factor V Leiden, prothrombin mutation, antithrombin, protein C, JAK2 V617F and CALR exon 9 mutations, as well as serum homocysteine). Her aspirin was switched to clopidogrel 75 mg daily. In October 2023, she was readmitted for another acute stroke, this time due to a distal M2 branch occlusion, again treated with intravenous thrombolysis.",
