  "abstract": "A 53-year-old man presented to hospital with a 4-week history of headache, nausea, vomiting and recurrent episodes of loss of consciousness associated with tongue biting. In the preceding 4 weeks, he had developed worsening unsteadiness on walking, slurred speech, mild confusion and twitching movements at night. There was no history of fever nor respiratory symptoms. He had lost weight due to poor oral intake. There was a history of hypothyroidism. He worked full time as a residential caretaker, drank minimal alcohol and did not smoke. He had grown up in Ecuador and had recently returned from a trip there.",
