  "abstract": "Background Super large-bore aspiration catheters may improve first-pass reperfusion and provide stable intracranial support during endovascular thrombectomy. We report early real-world performance and safety of the SOFIA 88 (0.088-inch inner diameter).Methods We performed a retrospective single-center consecutive case series of adults with LVO/DMVO treated with thrombectomy in which SOFIA 88 was used (March 1-November 11, 2025). SOFIA 88 was used as first-line direct aspiration (SOFIA 88-ADAPT) or as an intracranial support catheter for combined/rescue strategies. The primary endpoint was technical feasibility (successful intracranial use without abandonment due to access limitations). Secondary endpoints included modified first-pass effect (mFPE; mTICI 2b-3 after first pass) and final successful reperfusion (final mTICI 2b-3). Safety outcomes included catheter-related vessel injury, embolization to new territory (ENT), and symptomatic intracranial hemorrhage (sICH).Results Twenty-four patients were included (median baseline NIHSS 14); 87.5% (21/24) were anterior circulation. Technical feasibility was 100% (24/24); SOFIA 88-ADAPT was first-line in 37.5% (9/24). Overall mFPE was 62.5% (15/24) and final mTICI 2b-3 was 87.5% (21/24); in the SOFIA 88-ADAPT subgroup, mFPE was 77.8% (7/9). Median puncture-to-reperfusion time was 20 minutes (IQR 13-36). No dissections/perforations or ENT occurred; sICH occurred in 12.5% (3/24).Conclusions In this early single-center experience, SOFIA 88 use was technically feasible with high first-pass and final reperfusion rates and no device-related vessel injury.
