  "abstract": "Introduction Recanalization of large vessel occlusion by thrombectomy or stenting of stenotic carotid disease exposes the patient to high risk of reperfusioninjury. We devised a paradigm to tailor goals in blood pressure (BP) control post reperfusion therapy, assessing risk of both hypo- or hyper-perfusion, using transcranial doppler, near infra-red spectroscopy and quantitative EEG to gauge both risks and adapt the lower and upper limit of our targeted range of acceptable BP.Methods Systematic review of published and conference data reporting on physiologic assessments and risk stratification of these risks of both hypo- or hyper-perfusion.Results We identified that SBP being too high may lead to shearing injuries risking reperfusion injury, SAH, edema or hemorrhagic transformation. Pulsatility, resistivity and peak systolic velocities interhemispheric indexes, as well as TIBI flow pattern classification and certain quantitative EEG findings can help gauge this risk. Low end-diastolic velocities, blunted or dampened waveforms and low O2 on spectroscopy predict inadequate hypoperfusion.Conclusions We propose to target SBP < 140 instead of < 180 mmHg in hyperperfusive high risk patients, and to elevate the lower limit of the range to > 120 instead of 100 mmHg in hypoperfusive high risk patients. All combined with glycemic control 100-140 g/dL and temperature control [36.5-37.5] Celsius, to individualize BP targets and minimize both ischemic and edematous or hemorrhagic complications.Disclosures B.D.
