  "abstract": "Introduction The Woven Endobridge (WEB) device is an established and effective way to secure wide-neck aneurysms. However, occlusion rates vary widely and we sought to identify predictors of aneurysm recurrence.Methods We reviewed 25 patients treated between January 2020 and December 2024 with one WEB device. Data was collected on clinical presentation, aneurysm characteristics, WEB device size, and details of procedure.Results We identified 25 patients with wide neck aneurysms (aneurysm neck > 4 mm and/or dome:neck ratio < 2). Most aneurysms were located at the middle cerebral artery bifurcation (n = 13) and anterior communicating artery (n = 8). Others were at the basilar tip (n= 2), fetal posterior cerebral artery (n = 1), and origin of the posterior inferior cerebellar artery (n = 1). Patients were treated with a single layer WEB device ranging from 4-9 mm by 2-4 mm, the most used size being 5 x 2 mm. 19 of the 25 patients received WEB devices larger than or equal to the greatest dimension of the aneurysm. Two patients required retreatment with stent-assisted coiling. Notably, both patients were treated with a WEB device smaller than the greatest dimension of the aneurysm, with device sizes 7 x 3 mm and 8 x 3 mm for aneurysm diameters of 7.2 mm and 8.1 mm respectively. These aneurysms were wider (z-scores: 1.42 and 1.07), and one was greater in diameter (z-score: 1.63), depth (z-score: 2.305), and dome:neck ratio (z-score: 2.06) compared to the respective means.Conclusion Single layer WEB devices were used to treat 25 patients with wide neck aneurysms. In the two instances of aneurysm recurrence requiring retreatment, the WEB was smaller in width than the aneurysm itself identifying under sizing as a potential risk factor.Disclosures E. Tao: None.
