  "abstract": "Background Mycotic aneurysms (MAs) are uncommon and potentially fatal vascular issues that arise from systemic infections, most frequently linked to infective endocarditis. Much of the current literature focuses on ruptured MAs due to their urgent nature, but the optimal management approach for unruptured MAs remains unclear. Specifically, the effectiveness of endovascular treatment (EVT) for unruptured MAs has not been thoroughly assessed. To address this gap, we conducted a systematic review and meta-analysis to evaluate the efficacy and safety of EVT in unruptured MAs, focusing on technical success, angiographic results, complication rates, functional outcomes, and mortality.Methods An extensive search of PubMed, Scopus, EMBASE, and Web of Science was performed through January 2026 to find studies reporting on the outcomes of EVT in unruptured MAs. Titles, abstracts, and full texts were screened using Rayyan, and data was collected in Microsoft Excel. We included studies that clearly differentiated unruptured cases and reported relevant outcomes. A proportional meta-analysis utilizing a random-effects model was carried out in R version 4.4.2. Publication bias was evaluated using a funnel plot.Results Eight studies involving 43 unruptured MAs satisfied the inclusion criteria. EVT achieved a 100% success rate in technical procedures and complete occlusion at follow-up (95% CI: 95-100%). Complications related to the procedure were rare, occurring in 4% of the cases (95% CI: 0-17%). Functional outcomes were positive, with 90% of patients reaching either a modified Rankin Scale score of 0 or a Glasgow Outcome Score of 5 at follow-up (95% CI: 74-100%), indicating exceptional neurological recovery. No fatalities were reported among unruptured cases (0%; 95% CI: 0-13%).Conclusion EVT represents a safe and effective treatment option for unruptured MAs, demonstrating high technical success, lasting occlusion, low complication rates, and excellent functional outcomes. However, given the infrequency of unruptured cases, additional large-scale and prospective studies are necessary to confirm these conclusions and refine management strategies.Disclosures T.
