  "abstract": "Introduction/Purpose Intracerebral hemorrhage (ICH) carries high mortality and long-term disability worldwide. Prior Global Burden of Disease (GBD) analyses demonstrated a disproportionate burden in rapidly modernizing, middle-income countries, particularly in East Asia and the Pacific. We used updated 2023 GBD data to reassess global ICH burden and identify evolving socioeconomic and regional trends.Materials and Methods We used the updated GBD database to extract mortality and years lived with disability (YLD) per 100,000 individuals attributable to ICH across all countries from 1990 to 2023. We collected gross domestic product (GDP) per capita, United Nations geoscheme region, Healthcare Access and Quality Index (HAQI), and neurosurgeons per capita when available.Results 17/20 (85%) countries with the highest ICH death burden ( figure 1) fell within the middle 50% of GDP per capita, while 13/20 (65%) countries with the highest YLD burden (figure 2) fell within this range. In contrast to prior analyses, 10/20 (50%) countries with the highest death burden and 14/20 (70%) with the highest YLD burden were located in Polynesia, Micronesia, and Melanesia. From 1990 to 2023, countries demonstrated an initial increase in ICH burden followed by a decline with increasing economic development.Conclusion Updated 2023 data demonstrate a geographic shift in ICH burden from East Asia toward Pacific Island regions, while middle-income countries continue to carry disproportionate risk. Rapid epidemiologic transition, rising vascular risk factors, and limited access to specialized care likely contribute. These findings highlight the need to expand neurointerventional access, strengthen stroke systems, and implement targeted prevention strategies in emerging high-burden regions.Disclosures M.
