  "abstract": "Background The reporting of dural arteriovenous fistulas (dAVF) in the pediatric population is limited to small and single center studies. We aimed to analyze the clinical presentation, angioarchitectural features, and outcomes of dAVF in pediatric patients using the largest multicentric cohort of dAVFs available.Methods The Consortium of Dural Arteriovenous Fistulas Outcomes Research (CONDOR) database was analyzed. The registry involved 2066 dAVFs of 14 centers collected from 1990 to 2026. Patients with ages ranging from 0 to 18 years old were included. Demographics and clinical presentation were retrieved from electronical medical records. Treatment modality and angioarchitectural characteristics including Borden type, fistula location, arterial feeders, venous drainage and cortical venous drainage (CVD) were obtained. Pediatrics dAVF were matched by location, Borden type, superficial and deep drainage to adults dAVF. Outcomes analyzed included hemorrhage at presentation and complete obliteration of the fistula on last follow-up. Multivariable logistic regression was conducted to determine the specific differences between pediatric and adult dAVFs.Results A total of 38 patients were included. Average age at presentation was 7.8 years old, 73.7% were male, 21.1% had a history of head trauma and 76.2% of patients were symptomatic. A total of 15.8% of cases had hemorrhage at presentation. The Borden type III was the most frequent angioarchitectural subtype in 52.6% of patients and CVD was present in 76.3% of cases. The middle meningeal artery was the most frequent arterial feeder in 26.7% of patients and the most common fistula location was in the superior sagittal sinus in 18.4% of cases. A total of 92.1% of patients underwent treatment, 89.5% had endovascular embolization and 2.9% open surgical procedures. From the endovascular cohort, 34.2% had angiographic cure on one session, and 55.3% had unsuccessful obliteration. Only 47.4% of fistulas had complete obliteration on the last imaging follow-up. There was no association between pediatric dAVF and hemorrhage at presentation (OR 0.55, 95% CI 0.18 - 1.63, p = 0.28) but these fistulas were less likely to have a complete closure (OR 0.32. 95% CI 0.12 - 0.84, p = 0.02) compared to dAVFs.Conclusion Pediatric dAVF are less likely to be completely obliterated compared to adult dAVF.Disclosures A.
