  "abstract": "Background Radial access has grown in utilization for Neurointerventional procedures, offering reduced access site complications and faster patient recovery. While aspiration thrombectomy via transradial access (TRA) has been published using either direct access or using 5-French or smaller intermediate catheters, the inner diameters of 7-French guide catheters have generally lacked compatibility with 6-French aspiration catheters. This limits the aspiration lumen of the intermediate catheter, negatively impacting suction force and diminishing recanalization efficacy. The Piraeus 7-French 87 Neuglide offers a new option as a 6-French-aspiration-catheter-compatible radial guide. Here, we present a series of transradial aspiration thrombectomy cases using the Piraeus 7F 87 NeuGlide catheter.Methods In our single center, retrospective case series, we included consecutive patients treated for large or medium vessel occlusions via right proximal TRA. In brief, the 87 NeuGlide was navigated through a slender 7-French sheath 23 cm in length. A FreeClimb 70, Zoom 7X, or FreeClimb 54 was then advanced over an inner delivery device to perform contact aspiration mechanical thrombectomy. Patient characteristics, lesion location, procedure time, eTICI, and mRS were noted.Results Fourteen patients were treated (six females, mean age 72.9±14.9), with successful delivery of the aspiration catheter to the occlusion in 13; conversion to transfemoral access occurred in 1 case. Mean NIHSS at admission was 16.4±8.9. Mean procedure time was 31.1±14.9 min; time to first target-vessel angiogram was 8.7±2.9 min. Two patients had tandem lesions. Occlusions were located in the carotid terminus in two patients, M1 in five, M2 in three, M3 in two, and the basilar artery in two. First pass effect eTICI 2b-3 was achieved in 78.6% (11/14) of patients. Mean number of passes was 1.6±1.0. Final mTICI 2b-3 was 100% (14/14), and mTICI 2c-3 was 78.6% (11/14). Premorbid mRS 0-2 was 71.4% (10/14). Among those, post-discharge mRS was 0-2 in 80.0% (8/10, 8/14 overall). Mortality was 28.5% (4/14), two from stroke burden and two from unrelated cardiopulmonary events. No distal emboli, vasospasm, or intraoperative adverse events were reported. Two patients experienced hemorrhagic conversion, one of which was symptomatic.Conclusion The Piraeus 7-French 87 NeuGlide catheter enabled effective navigation from proximal radial access to cervical arteries, supporting thrombectomy with a high rate of technical success and no device-related or access-site complications. While a case series has previously reported use of the Piraeus 7-French 87 NeuGlide for a wide range of procedures, and the Zebra 7-French has shown basic safety and technical success, this is the first case series for thrombectomy through the 7-French 87 NeuGlide and demonstrated very high rates of successful and complete recanalization. Given its lower access site risk, and with the limitation on intermediate catheter compatibility addressed, TRA should be considered as an option when treating large or medium vessel occlusions.Disclosures M.
