  "abstract": "Introduction The MDRinAIS study is an EU-funded initiative that aims to develop a telemedicine-enabled management pathway for patients with acute ischemic stroke (AIS) due to large vessel occlusion.Aim of Study To reduce treatment delays and improve AIS patient outcomes.Method This retrospective multicentric study included AIS patients treated with the ‘drip and ship’ model between January 2018 and December 2021 at three centers: Italy, Switzerland, and Spain. Patients were categorized by treatment type (mechanic trombectomy, fibrinolysis, or both) and modified Rankin Scale (mRS). Timing metrics (in minutes) included admission-to-groin puncture (DTG), admission-to-fibrinolysis initiation (DTIF), fibrinolysis-to-hub arrival (DoF), and admission-to-hub groin puncture (DT).Results 572 patients were enrolled (Spain: 22.38%; Italy: 37.41%; Switzerland: 40.21%). Significant differences in treatment distribution, times and complications were observed across centers (p<0.001). Italy’s (67.70±42.02) and Switzerland’s (55.97±104.5) DTG time was significantly lower than Spain’s (96.92±44.96; p=0.0031 and p<0.001, respectively). Similar differences were noted for DT, DTIF and DoF times. Post-treatment complications included: vessel dissection (Italy: 0.9%, Spain: 13.68%, Switzerland: 5.13%, p<0.001); embolism in new territories (Spain: 8.42%, Switzerland: 4.08%, p<0.001); and other complications (Italy: 3.3%, Spain: 24.21%, Switzerland: 14.35%, p<0.001); distalization rates showed no differences.
