  "abstract": "Introduction Brain arteriovenous malformations are one of the most complex and challenging intracranial vascular pathologies – regarding the anatomical and flow characteristics as well as the treatment strategies based on these. Recent technical developments in 3D virtual reality applications have become available now also for these kinds of especially demanding and complex cases in a clinical setting.Case Description We present the case of a 36-year-old male patient with a 12 previously radiosurgically treated (12 years ago) large right hemispheric high-flow brain AVM Spetzler Martin Grade IV. He suffered from severe headaches for two weeks and was diagnosed with fresh intraventricular bleeding. Multimodal imaging with MRI and DSA suggested the bleeding source as a ventricular hemorrhage due to a ruptured flow associated fusiform aneurysm of a PCA feeding artery branch, but the near relationship to adjacent feeders from the anterior choroidal artery made the diagnosis challenging. The additional 3D VR visualization tool (Specto Medical) was helpful in detecting the bleeding source and understanding the complex anatomical characteristics. The patient was successfully treated with a targeted aneurysm occlusion by parent/feeding artery occlusion and developed no new symptoms or rebleeding.Abstract C80 Figure 1Conclusions Especially in highly complex AVM or dAVF cases, the supporting use of 3D VR visualization applications might be extremely helpful in the treatment planning of these demanding vascular pathologies.
