  "abstract": "Introduction Hematoma recurrence and progression remain common challenge in the management of nonacute subdural hematomas (SDH).Aim of Study To identify radiographic features associated with hematoma recurrence or progression following middle meningeal artery (MMA) embolization and to explore potential radiographic biomarkers for selecting candidates for this treatment in nonacute SDH.Method The study of managing nonacute SDH using liquid materials: a Chinese randomized trial of MMA treatment (MAGIC-MT) was a multicenter, prospective, open-label parallel group trial with blinded outcome assessment. Patients with symptomatic nonacute SDH with mass effect were included. Patients were assigned to undergo burr-hole drainage or receive nonsurgical treatment at the surgeon’s discretion, and patients in each group were then randomly assigned to undergo MMA embolization (MMAE) or to receive usual care.Results 697 patients were finally included in this post hoc study. Among patients with the high-density capsule sign, MMAE was associated with a lower incidence of symptomatic recurrence or progression compared to usual care (5.0% vs. 12.0%, RR, 0.39 [95% CI, 0.21 to 0.72], p=0.009). Patients without the high-density capsule sign showed no significant difference in recurrence or progression between MMAE and usual care (9.4% vs. 5.6%, RR, 1.56 [95% CI, 0.62 to 3.89], p=0.240), with significant interaction (p for interaction = 0.014).Conclusion In patients with nonacute SDH who present with high-density capsule sign on non-contrast CT scan, adjunctive MMAE demonstrated lower incidence of symptomatic recurrence or progression and fewer serious adverse events compared to usual care.
