  "abstract": "Introduction Flow diverters have expanded endovascular treatment options for cerebral aneurysms, but comparative data regarding their efficacy and safety in distal anterior cerebral artery aneurysms are limited.Aim of study We aimed to compare clinical and radiographic outcomes among different FD devices in the treatment of unruptured DACA aneurysms.Method Multicenter cohort study involving 166 patients from 39 centers across 14 countries who underwent FD treatment for unruptured DACA aneurysms between 01/2018 and 12/2022. Patients were treated with PED, SVB, FRED Jr., or other FDs. The primary outcome was complete aneurysm occlusion at last imaging follow-up, assessed using the OKM classification. Secondary outcomes included procedural complications, retreatment rates, and mRS scores at last follow-up.Results The median age was 61 years, and 74% were women. Aneurysms were predominantly saccular and located on the pericallosal artery. Immediate complete obliteration occurred in 20% of cases, and adequate occlusion (OKM C-D) was achieved in the majority, with no significant differences among devices. Ischemic complications occurred in 11% of patients, hemorrhagic complications in 5%, and symptomatic permanent ischemia in 0.6%, without significant differences by device type. In-stent stenosis occurred in 17% of cases. The retreatment rate was 1.3%. At last follow-up, the median mRS score was 1.Conclusion Among patients with unruptured DACA aneurysms, treatment with PED, SVB, FRED Jr., and other flow diverters was associated with high rates of aneurysm occlusion, low rates of complications, and favorable functional outcomes.
