  "abstract": "Introduction Intracranial artery stenosis (ICAS) is a major cause of ischemic stroke. Stenting with percutaneous transluminal angioplasty (PTA) is an established treatment, and modern PTA and stent systems are being developed to improve outcomes.Aim of Study To compare the efficacy and safety of the pEGASUS-HPC and CREDO® heal stents, used with the NeuroSpeed® PTA balloon catheter, in treating severe symptomatic ICAS, with or without acute occlusion.Method In this retrospective, multicenter study (July 2021 to June 2024), patients with severe symptomatic ICAS treated with NeuroSpeed® combined with the pEGASUS-HPC or CREDO® heal stent were included. Demographic data, clinical parameters, procedural details, and outcomes were collected and analyzed.Results 69 patients were included (pEGASUS-HPC: n=34; CREDO® heal: n=35). Mean ages were 69.8±12.2 and 72.2±12.5 years, with 68% and 60% male patients, respectively. Both groups showed significant improvement in arterial diameter (0.47 to 1.69 mm; and 0.57 to 1.89 mm). Revascularization was successful in 88% and 94% of cases. The National Institutes of Health Stroke Scale (NIHSS) and the modified Rankin Scale (mRS) scores improved significantly in both groups. Retreatment was required in 12% vs. 6%. In-stent stenosis occurred in 15% vs. 6%, and in-stent thrombosis in 9% vs. 6%, respectively.Conclusion Both the pEGASUS-HPC and CREDO® heal stent systems, used with the NeuroSpeed®, appear safe and effective for treating severe symptomatic ICAS. Outcomes were comparable, though the CREDO® heal stent showed a slight advantage in reducing in-stent stenosis and thrombosis.
