  "abstract": "Introduction Carotid-cavernous fistulas(CCFs)are abnormal arterio-venous communications in the cavernous sinus.Recent advances in endovascular technology have made available a number of different treatment options for CCFs.Aim of Study We conducted this study to determine the safety and efficacy of embolization of carotid-cavernous fistulas with Onyx.Method We retrospectively collected data in all patients with CCFs who underwent Onyx-based embolization at our institution over a 4-year period from January 2017 to January 2021, with recording of the type of fistula, route of embolization, additional use of coils, technical success, inadvertent events, procedural complications, recurrence rate and clinical follow-up.Abstract A184 Figure 1Results A total of 26 patients underwent embolization using Onyx. Embolization was performed via a trans-venous route in 12 cases and a trans-arterial route in 14 cases.Onyx 18 was used in all cases.Additional embolization with framing coils was performed in 25 cases.All procedures were completed in a single session.Immediate fistula obliteration was achieved in all cases.Clinical improvement of presenting symptoms occurred in 100% of the patients by 3 months.Neurological complications occurred in 3 patients.Two patients developed a transient VI cranial nerve (CN) palsy that resolved within 3–4 months.One patient developed VII CN palsy that had minimal improvement on follow-up.Abstract A184 Figure 2Conclusion Onyx is a liquid embolic agent that is effective in the treatment of CCFs.There is no absolute theoretical contraindication to use Onyx for treating CCFs, whereas despite efficacy and benefits of Onyx in the treatment of CCFs, it is not without side effects.Therefore good selection of patients is needed and technical tips and tricks should be respected.Conflict of Interest No",
