  "abstract": "Introduction Intracranial arteriovenous malformations (AVMs) are rare, however, severe cerebrovascular pathologies that often present to emergency departments (EDs) with hemorrhage or seizures. Understanding discharge outcomes is essential for effective patient management and efficient resource utilization.Aim of Study To identify predictors of non-home discharge among patients with intracranial AVMs admitted through the ED, using national inpatient database, and to investigate impact of clinical presentation and treatment modality.Method We queried the Nationwide Inpatient Sample (2016–2022) to identify patients with a diagnosis of brain AVM (ICD-10: Q28.2) who were admitted through the ED. The primary outcome was non-home discharge. Multivariable logistic regression was performed to determine independent predictors, including demographics, clinical presentation (rupture or seizure), early intervention, treatment modality (surgical or endovascular), length of stay (LOS), and APR-DRG severity and risk scores.Results Among ED-admitted AVM patients, following features were independently associated with higher odds of non-home discharge: age (OR 1.04, 95% CI 1.04–1.04), ruptured presentation (OR 1.66, 95% CI 1.43–1.93), seizure (OR 1.16, 95% CI 1.02–1.31), early intervention (OR 1.56, 95% CI 1.16–2.11), LOS (OR 1.05 per day, 95% CI 1.04–1.06), APR-DRG mortality risk (OR 1.38, 95% CI 1.27–1.51), and APR-DRG severity (OR 1.44, 95% CI 1.28–1.62). Whereas, endovascular interventions were independently associated with reduced odds of non-home discharge (OR 0.57, 95% CI 0.43–0.76).Conclusion Older age, ruptured or seizure presentation, early intervention, longer stay, and higher APR-DRG scores increased the odds of non-home discharge.
