  "abstract": "Introduction Basilar artery stenosis is a high-risk subset of intracranial atherosclerotic disease, often associated with devastating posterior circulation strokes. Despite aggressive medical therapy, some patients continue to experience recurrent ischemic events. In such cases, endovascular intervention may be a viable option. We report a case of symptomatic high-grade basilar stenosis treated successfully with angioplasty and stenting.Aim of Study To highlight the role of endovascular revascularization in medically refractory basilar artery stenosis and discuss technical considerations for safe and effective interventionMethod A 62-year-old diabetic female presented with recurrent posterior circulation TIAs and a prior occipital-cerebellar infarct despite being on dual antiplatelet therapy and high-dose statins. Digital subtraction angiography revealed severe focal basilar artery stenosis. Under general anesthesia, endovascular intervention was performed via transfemoral approach. The lesion was pre-dilated with a Neurospeed balloon, followed by deployment of a Credo stent. Intraprocedural imaging confirmed good stent apposition and vessel patency. Antiplatelet therapy was optimized pre- and post-procedure.Results The procedure was uneventful, with successful restoration of luminal diameter and improved distal flow. The patient remained neurologically stable post-intervention and is under clinical and imaging follow-up.Conclusion While basilar artery stenting carries inherent risks, especially involving perforator-rich zones, careful patient selection, lesion preparation, and device choice can lead to favorable outcomes.
