  "abstract": "Introduction Aspiration thrombectomy to treat AIS is typically performed by generating a constant vacuum with a syringe or electronic pump. Cyclic aspiration, combining high frequency pulsed vacuum forces, is thought to improve aspiration efficiency.Aim of Study This systematic literature review aims to compare the outcomes of standard and cyclical aspiration in pre-clinical and clinical settings.Method A systematic literature review of studies published by April 2025 was conducted using the Nested Knowledge platform. The primary outcome was a comparison of FPR defined as mTICI2c-3 in one pass clinically or complete clot removal in one pass pre-clinically. Results were compared using a two-sample Student’s t-test. The pre-clinical rates were compared to clinical outcomes as a measure of reliability for predicting outcomes. Out of 962 articles, 8 pre-clinical and 3 clinical articles were included. All pre-clinical studies were in vitro (n=542), and all clinical studies were in humans with LVOs (n=96).Results Pre-clinical (45±20.1% vs 84.7±8.6%, p=0.14) and clinical (33.3±5.3% vs 63.7±6.5%, p=0.04) FPRs were greater with cyclic vs standard aspiration. Although non-significant, overall FPR was observed 35% lower clinically vs pre-clinically. Pre-clinical experiments suggested an 89% increase in FPR with cyclic aspiration. This was well-matched in the clinical studies which showed improved outcomes by 94%. The pre-clinical ENT count with cyclic aspiration was greater vs the clinical rate (10±7.9% vs 1.3±1.3%, p=0.4).Conclusion Although the pre-clinical evidence reported non-significant higher rates of FPR and ENT, these data suggest a high ability for the pre-clinical studies to predict the additional benefit of cyclic aspiration clinically.Conflict of Interest No",
