  "abstract": "Introduction Balloon Test Occlusion (BTO) was performed selectively and involved the following steps: 1) a guiding catheter placed in the common carotid artery; 2) a balloon catheter, Scepter C (MicroVention, Tustin, California) (4x11mm), placed at the immediately before the ophthalmic segment of the ICA, including OA origin; 3) careful inflation of a balloon with 100% contrast media under fluoroscopic observation for 20 minutes; 4) contrast instilled through the common carotid artery to evaluate collateral flow between the external carotid artery and the OA.Case Description A ‘pass’ result with BTO was defined as retrograde filling of the OA main trunk via ECA branches and demonstration of retinochoroidal blush without visual and neurological deteriorations. In BTO-’pass’ patients, occlusion of the aneurysm and compromise of the OA orifice could be attempted in the treatments.1. 70-year-old female. Medial directed 6.2x5.3mm sized left OA aneurysm patient passed BTO. S microcatheter (MC) was used to preserve OA and stent assist coil embolization (SAC) was performed without complications.2. 61-year-old female. Superior directed 3.6x3.5mm sized right OA aneurysm patient passed BTO. Using Straight & Pig tail MC, stent assist coil embolization was successfully performed while preserving OA orifice.3. 58-year-old female. Medial directed 3.0x2.8mm sized left OA aneurysm patient. BTO failed and no treatment was performed.Conclusions In patients with OA aneurysm incorporating an OA origin, BTO may be helpful in devising treatment strategies to preserve vision when coiling aneurysms.
