  "abstract": "Introduction Frailty is increasingly acknowledged as a determinant of stroke outcome. Tools such as the Clinical Frailty Scale (CFS) and modified Rankin Scale (mRS) are often used to estimate baseline function, but their prognostic utility in patients undergoing Endovascular Thrombectomy (EVT) is poorly defined.Aim of Study To evaluate the ability of established frailty indices and related clinical variables to predict functional outcomes following EVT.Method We prospectively analysed a cohort of 237 consecutive stroke patients who underwent EVT. Predictors included baseline mRS, CFS, and binary frailty-related variables (polypharmacy, cognitive impairment, recent hospitalisation, mobility decline, underweight status, and comorbidities). The primary outcome measure was functional independence at 90 days (mRS ≤2). Predictive performance was assessed using univariate logistic regression and ROC analysis.Results Baseline mRS (AUC 0.25) and CFS (AUC 0.27) demonstrated poor discrimination for predicting 90-day functional independence. Several individual frailty indicators showed significant univariate associations with poor outcome, including pre-stroke cognitive impairment (p = 0.01), recent hospital admission (p = 0.02), and mobility decline (p = 0.001). However, none of the assessed indices or binary markers provided clinically useful predictive accuracy when used in isolation. Predictive power remained weak even when combining multiple frailty variables.Conclusion Standard frailty metrics such as mRS and CFS perform poorly in predicting post-EVT functional outcomes.
