  "abstract": "Introduction Flow diverter (FD) treatment for acutely ruptured intracranial aneurysms (IA) carries notable risks of treatment-related complications, despite generally favorable neurological outcomes. We previously reported the national rates of both favorable outcomes and associated complications. Factors related to favorable neurological outcomes, as well as treatment-related complications have not been studied in depth.Aim of study In this study, we further analyzed the data to identify factors associated with favorable outcomes, as well as those linked to post-operative complications.Method Between January 2012 and December 2019, 110 patients with acutely ruptured intracranial aneurysms (IAs) were treated with FDs. The data on treatment, complications, and patient outcomes were retrospectively collected. A logistic regression model was used for both univariate and multivariable analyses.Results In the univariate analysis, age <65, favorable WFNS score, no post-operative stroke, pre-operative EVD, and aneurysm treatment later than >7 days of ictus were identified as predictors of better neurological outcomes. However, in the multivariable analysis, only age <65 (p=0.0065), pre-operative EVD (p=0.011), and aneurysm treatment later than >7 days of ictus (p=0.004) were statistically significant. Concerning intracranial hemorrhagic complications, only post-operatively placed EVD (p=0.012) was significant in both univariate and multivariable analyses. As for post-operative ischemic stroke, only in-stent thrombosis (p=0.006) reached statistical significance in our analysis.Conclusion By considering factors such as patient age, timing of treatment, and the preoperative placement of an EVD, the risks associated with FD treatment for acutely ruptured IAs may be reduced, potentially leading to more favorable neurological outcomes.Conflict of Interest No",
