  "abstract": "Introduction Intracranial aneurysms arising from the posterior inferior cerebellar artery (PICA) are often a therapeutic challenge due to the anatomical complexity, small size and potential involvement of brainstem perforators. Endovascular techniques, including adjunctive devices, are increasingly employed as first-line treatment options. We present a case of a ruptured small distal PICA aneurysm treated successfully using endovascular coiling with Comaneci temporary stent assistance.Abstract C70 Figure 1Case Description 65-year-old male presented to the ED with sudden-onset severe headache and vertigo. Admission scores were: Hunt-Hess: Grade III, WFNS: Grade IV. neCT and CTA revealed SAH in the posterior fossa and a small (2.1 × 2.0 × 1.9 mm neck 2.0 mm) saccular aneurysm at the origin of the right PICA (Fig.1). The aneurysm gave rise to the parent artery (PICA), which limited the feasibility of simple coiling. Endovascular embolization was performed using Axium soft detachable coils and a temporary bridging stent Comaneci 17 (Fig.2.). After stent removal, intraprocedural short-segment thrombosis of the PICA occurred. Intra-arterial bolus of eptifibatide was administered, leading to complete recanalization. Final angiographic control confirmed stable coil placement and preserved flow through the PICA. Postprocedural imaging confirmed complete aneurysm embolisation, preserved PICA flow and no ischemic changes (Fig. 3A-C).
