  "abstract": "Introduction Large vessel occlusion (LVO) stroke is associated with high morbidity and mortality. Achieving first pass effect (FPE) improves outcomes. The 6×50 mm APERIO® Hybrid21 stent-retriever combines design features to optimize clot engagement and retrieval, yet real-world data on its performance remain limited.Aim of Study To assess the procedural efficacy and safety of the 6×50 mm APERIO® stent-retriever in patients with LVO stroke at a high-volume tertiary center.Method We retrospectively analyzed 127 consecutive patients treated between September 2023 and July 2024. Primary endpoints included FPE rate, final TICI score, and symptomatic intracranial hemorrhage (sICH). Secondary outcomes included early neurological improvement (ENI), modified Rankin Scale (mRS) at follow-up, and multivariable predictors of outcome.Results FPE was achieved in 55%, with successful reperfusion (TICI ≥2b) in 85% and complete reperfusion (TICI 3) in 36%. ENI occurred in 21% of patients. sICH was observed in 3.1%, and embolization to new territory in 4.1%. At 90-day follow-up, 41% achieved functional independence (mRS 0–2); mortality was 18.9%.
