  "abstract": "Introduction Complex vessel anatomy and challenging neurovascular interventions that require tip shaping for guidewires add unnecessary time and inefficiencies to these procedures. The SmartGUIDE (Artiria Medical, Geneva Switzerland) guidewire was developed to overcome these challenges with a dynamic deflectable tip that can be locked in 3 progressive positions.Aim of study SmartRETRO, a retrospective post market study, was conducted to evaluate the performance of SmartGUIDE to successfully navigate to the target location without the need for shaping, removal or exchange of the wire.Method Fifty neurointerventional procedures at 5 US centers were performed using the 0.014’ SmartGUIDE guidewire. Retrospectively collected data included clinical outcome, technical success, procedure time, and periprocedural complications (thromboembolic, hemorrhagic, vessel dissection or perforation, infection).Results Overall, fifty procedures were performed where SmartGUIDE was used, including carotid stenting, unruptured aneurysm treatment including flow diversion, intrasaccular embolization and coiling, intracranial venous stenting and middle meningeal artery embolization. The wire was able to successfully reach the target lesion in all cases without the need to exchange, reshape or hand shape the wire. Besides shape retention, torquability, pushability and trackability were favorable in all cases. No compatibility issues were found between SmartGUIDE and a variety of 0.017’- 0.027’ micro and balloon guide catheters. No device-related complications were recorded intraoperatively or at patient discharge.Conclusion The initial results from SmartRETRO demonstrate the excellent performance and safety of SmartGUIDE when used in various neurointerventional procedures, along with its diverse compatibility with current devices.
