  "abstract": "Introduction Intrasaccular devices such as the Woven EndoBridge (WEB) have proven to be safe and effective for the treatment of intracranial aneurysms. However, optimal sizing remains a matter of debate.Aim of study This study evaluates the feasibility of a simplified sizing strategy using a ‘shallow’ WEB device for the treatment of intracranial ruptured and unruptured aneurysms.Method This single-center retrospective study included patients treated with a shallow WEB device between March 2015 and August 2024. A shallow WEB was defined as a device with the smallest available height for a given width, and a dome-to-WEB height ratio greater than 1.5. Anatomic results were assessed by digital subtraction angiography (DSA) immediately post-procedure, at 6 months, and at 1 year. Occlusion rates were compared between ruptured and unruptured aneurysms, and statistical differences between groups were evaluated using the chi-square test.Results 90 patients with 93 aneurysms were treated with a shallow WEB device. Of these, seventy-nine had 1-year DSA follow-up, and twelve had 6-month DSA follow-up. 33 aneurysms (35.5%) were ruptured, and 60 aneurysms (64.5%) were unruptured. At final DSA, complete aneurysm occlusion (WOS A+B) was achieved in sixty-sever aneurysms (72%), neck remnants (WOS C) were observed in twenty-one aneurysms (22.6%), and aneurysm remnants (WOS D) in five aneurysms (5.4%).
