  "abstract": "Introduction Endovascular treatment of wide-neck bifurcation intracranial aneurysms remains challenging. Approaches including intrasaccular devices, flow diversion, and stent-or balloon-assisted coiling have shown variable efficacy. The Contour Neurovascular System is a novel intrasaccular flow-diverting device for wide-neck bifurcation aneurysms. Adjunctive coiling via a jailed microcatheter may enhance thrombosis and device stability.Aim of study We present our single-centre experience with Contour-assisted coiling, evaluating safety and efficacy.Method A retrospective review was conducted on consecutive patients treated with Contour-assisted coiling at a tertiary neurovascular centre between May 2022 and August 2024. Demographics, procedural details, and clinical outcomes were analysed. Radiological outcomes were assessed using the modified Raymond-Roy classification on post-procedural and follow-up imaging (DSA, CTA, or MRA), categorised as complete, adequate, or incomplete occlusion. Safety outcomes included device-related complications, hospital stay, and modified Rankin Scale score at 6-month follow-up.Results Twelve patients underwent treatment with Contour-assisted coiling. Mean age at intervention was 67.7 years. Eleven aneurysms were unruptured. Mean neck width was 6.9 mm, dome height 10.8 mm, and maximum dome width 10.1 mm, with a dome-to-neck ratio of 1.6. Adequate occlusion (Raymond-Roy grades I–II) was achieved in 10/12 (83%) aneurysms, with complete occlusion in 8/12 (67%) at latest follow-up (mean 396 days). Two patients experienced minor periprocedural ischaemic events, both resolving without neurological deficit. No other complications were reported.Conclusion Contour-assisted coiling appears to be a safe, effective strategy for treating large wide-neck bifurcation aneurysms, offering good occlusion rates with low complication risks.
