  "abstract": "Introduction The optimal endovascular approach for posterior circulation stroke remains debated.Aim of Study This meta-analysis compares the efficacy and safety of aspiration and stent retriever techniques across key clinical outcomes.Method A comprehensive meta-analysis of 27 studies involving 3,157 was conducted. Primary outcomes included favorable functional outcome at 90 days (modified Rankin Scale ≤2) and successful recanalization. Secondary outcomes included first-pass effect, symptomatic intracranial hemorrhage (sICH), procedural complications, and mortality. Risk ratios (RRs) and adjusted risk ratios were pooled using fixed- or random-effects models, and heterogeneity was assessed using I² and χ² statistics.Results Stent retrievers were associated with a significantly higher likelihood of achieving a favorable outcome at 90 days (RR: 1.12, 95% CI: 1.02–1.24, p = 0.02; I² = 0%) and demonstrated a modest but significant advantage in successful recanalization (RR: 1.04, 95% CI: 1.01–1.08, p = 0.02; I² = 28%). No significant difference was observed in the first-pass effect though the trend numerically favored aspiration. sICH rates were also similar between techniques. Aspiration was associated with significantly fewer procedural complications. Mortality did not differ significantly between the groups.Conclusion Stent retrievers demonstrated superior efficacy in achieving favorable functional outcomes and successful recanalization in posterior circulation stroke, with a possible trend toward lower mortality. While complication rates were lower with aspiration, both approaches showed similar rates of symptomatic intracranial hemorrhage. These findings highlight the central role of stent retrievers in posterior circulation stroke and underscore the importance of technical proficiency and learning curve optimization to minimize procedure-related complications.Conflict of Interest No",
