  "abstract": "Introduction Chronic low back pain is a major cause of disability worldwide. Periradicular therapy is effective when conservative treatments fail. Fluoroscopy and CT are standard guidance tools but involve radiation exposure. MRI-guided interventions offer high soft-tissue contrast and no radiation, making them increasingly attractive. Recent low-field MRI systems, such as the 0.55-T Magnetom Free.Max, with large bore sizes and real-time imaging, present new opportunities for safe and precise spinal interventions.Aim of Study This study aimed to evaluate the accuracy and feasibility of real-time 0.55-T MRI-guided lumbosacral periradicular injections. Results were compared to CT-guided procedures with and without a laser navigation system, assessing technical success, needle positioning quality, puncture time, and overall intervention duration in a controlled phantom model.Method Five radiologists with varying experience levels performed nine randomized PRTs each on a lumbar spine phantom under MRI, CT with laser, and conventional CT guidance. Accuracy was assessed by two senior neuroradiologists using a 5-point scale. Puncture and total procedure times were recorded. MRI procedures used real-time BEAT IRT TRUFI sequences; CT-guided procedures were conducted with identical technical parameters.Abstract A354 Figure 1Results All procedures were technically successful. Needle placement quality was high across all modalities, with MRI scoring slightly better.
