  "abstract": "Introduction Cerebral vasospasm following subarachnoid hemorrhage (SAH) may cause cerebral ischemia. Typical management strategies include oral nimodipine, induced hypertension, and endovascular interventions like vasodilator infusion, balloon- or stent- angioplasty. However, mechanical treatments have limitations, including risk of vessel injury and limited feasibility in distal vasospasm.Aim of study To introduce a novel approach of mechanical vasospasm treatment.Method We evaluated the angioplastic effect of just navigating a J-shaped microwire through vessel segments severely affected by vasospasm in consecutive SAH patients. Multimodal angiographic and clinical results are recorded.Results Three consecutive patients with symptomatic vasospasm in the anterior circulation were treated with J-Wire Plasty (JWP)-technique using a 0.014’ Traxcess guidewire (Terumo, Irvyne, USA). Affected segments were M1/M2 segments of the middle cerebral artery and A1/A2/A3-segments of the anterior cerebral artery. The wire was preshaped before introduction or during its course through the carotid siphon and navigated through the affected arteries. In total, 8 segments in 3 patients were treated without other mechanical manipulation. Intra-arterial Nimodipine was added only after JWP was performed. All patients underwent control DSA with no recurrences in the treated segments.Conclusion J-Wire Plasty (JWP) without balloon or stent angioplasty may be a sufficient mechanical treatment in severe symptomatic cerebral vasospasm in selected patients, possibly reducing the risk and increasing the availability of mechanical vasospasm treatment.
