  "abstract": "Introduction Endovascular thrombectomy (EVT) is the standard of care for acute ischemic stroke in eligible patients. The size, structure, and positioning of guiding catheters are key factors affecting clot retrieval success and clinical outcomes.Aim of Study This study aimed to assess the impact of intracranial positioning of 8 French (F) guiding catheters on procedural time, first pass effect, angiographic success, and clinical outcomes, compared to 6F catheters.Method Patients aged ≥18 years, presenting within 6–8 hours of symptom onset, with NIHSS ≥5, pre-stroke mRS 0–2, and internal carotid artery (ICA) or middle cerebral artery (MCA) occlusion were included. Data on occlusion site, collateral score, puncture-to-recanalization time, eTICI score, number of passes, first pass recanalization, post-procedural complications, discharge mRS, and 3-month mRS were collected.Results First pass recanalization was achieved in 72.3% of the 8F group versus 43.7% in the 6F group (p<0.005). Recanalization within 45 minutes occurred in 72.3% of the 8F group compared to 52.1% of the 6F group (p<0.05). In petrous segment placements, the 3-month good outcome (mRS 0–2) rate was 59.4% with 8F catheters versus 18.8% with 6F (p<0.05).Conclusion Intracranial positioning of 8F guiding catheters was associated with higher first-pass success, shorter procedure time, and better functional outcomes.
