  "abstract": "Introduction Proximal basilar artery aneurysms present significant technical challenges due to complex anatomy, wide necks, and high-flow hemodynamics. When accompanied by a large BA diameter (>9.0 mm), conventional stent-assisted coiling may result in inadequate neck coverage or poor wall apposition. The kissing stent technique has been adapted to address these issues by providing dual stent support from bilateral VA converging into the basilar trunk, thereby creating a stable scaffold for coiling and optimizing flow dynamics.Abstract C61 Figure 1Case Description A 48-year-old male presented with diplopia and dizziness. DSA confirmed a vertebrobasilar dolichoectatsia with fusiform large aneurysm at proximal BA, above the vertebrobasilar junction with bilateral dominant vertebral arteries. To achieve stable neck coverage and coil support, a kissing stent technique was performed. Two Neuroform Atlas stents (4.5 mm × 30 mm) were deployed simultaneously—one from the right vertebral artery to basilar artery, the other from the left vertebral artery to basilar—forming a reverse Y-configuration at the vertebrobasilar junction. Through the jailed microcatheter, the aneurysm was densely packed with detachable coils. Final angiography showed complete occlusion with excellent stent apposition.Abstract C61 Figure 2Conclusions In large-diameter proximal basilar artery aneurysms, achieving proper stent apposition and neck protection is critical. The kissing stent configuration provides a more robust scaffold compared to single stent-assisted techniques.This approach also allows bilateral vertebral artery access, preserving posterior circulation flow dynamics.In this case, the technique provided durable aneurysm exclusion and preserved flow in all parent vessels.
